Most root canals are handled in-house at Hacton Dental Care with modern rotary technique. Specialist referral is reserved for failed retreatments, calcified canals, fractured instrument retrieval and apicectomy work where the extra precision genuinely changes the outcome.

When is a specialist endodontist needed?

A specialist endodontist becomes the right route when:

  • A previous root canal has failed and retreatment is needed
  • The tooth has calcified canals that cannot be located with standard equipment
  • A previous treatment has left a fractured file inside the canal
  • The root canal anatomy is unusually complex (curved roots, extra canals)
  • Apicectomy (microsurgical root-end surgery) is needed
  • You want the highest possible chance of saving a tooth before considering extraction

How much does specialist endodontics cost?

Specialist endodontics is more expensive than routine root canal treatment because of the equipment, expertise and longer appointment time involved. For complex cases this is genuinely worth it - the alternative is often losing the tooth.

Specialist endodontists work under a high-magnification operating microscope, allowing them to see and treat anatomy that's invisible to the naked eye. This is the single biggest difference between routine and specialist root canal work.

Root canal retreatment

Where a previous root canal has failed (persistent infection, new symptoms, radiographic findings), the specialist removes the existing filling, locates and treats any missed canals, and refills with modern materials. Success rates above 80% for retreatment cases.

Calcified canal treatment

Some teeth (particularly older patients, previously traumatised teeth) have canals that have partially calcified and become invisible on standard x-rays. The specialist's microscope and ultrasonic instruments can locate and open these.

Fractured instrument retrieval

Where a previous treatment has left a piece of broken file inside the canal, specialist instruments can sometimes retrieve it without compromising the tooth.

Apicectomy (root-end microsurgery)

For teeth where retreatment is not possible (existing crown that can't be removed, post-cemented in canal, persistent infection), apicectomy removes the infected tip of the root surgically while preserving the rest of the tooth.

Extraction is sometimes the right call. Some teeth have reached a point where saving them isn't realistic. But for most teeth with complex root canal needs, the specialist route gives a significantly better long-term outcome than extraction followed by an implant or bridge replacement.
